Community Partners Programme (CPP)

This program aims to assist older people from several culturally and linguistically diverse communities who are moving from home to residential aged care facilities. It offers:-
  • information about various types of aged care services and how to access them,
  • information about what aged care facilities would be suited to their individual needs,
  • any help needed to move from home to residential aged care,
  • support once the move from home into residential care has been made, and
  • information to staff of residential aged care facilities about cultural, spiritual and language needs.

Bi-cultural Workers
The Migrant Resource Centre's CPP program has access to our aged care staff who can speak Chinese, German, Philippine, Vietnamese, Bahasa Malay, Serbian, Polish, Dutch, and Spanish.

Community Support
Our Community Worker can visit communities or groups and talk about:
  • which residential aged care facilities in Tasmania incorporate different languages and cultural activities in their programs, and would therefore suit community members;
  • how to access these aged care services;
  • support that we can offer to the community members during the transition period between home and the aged care facility;
  • how to involve your community in the lives of members who are living in residential aged care facilities; and
  • set up links between your community and appropriate aged care services.

Aged Care Service Providers
For aged care service providers who wish to provide a better service for culturally and linguistically diverse communities we can:
  • help to identify the cultural, linguistic and spiritual needs of clients;
  • assist you to incorporate residents’ language and culture into your programs;
  • provide culture-specific cross cultural awareness training;
  • facilitate community input into the provision of culturally appropriate care; and
  • facilitate the creation of networks between your aged care service and communities.
