Linking Community to Residential Aged Care

Providing help and support to elderly migrants

Community Partners Programme (CPP)

The Community Partners program assists older people from culturally and linguistically diverse communities who are needing elderly care support in their home or are moving from home to residential aged care facilities. This program is provided in particular to the Chinese, German, Philippine, Hungarian and Ukrainian communities.

Providing information and resources about Aged Care

Some of the services the Migrant Resource centre offer are:

  • Aged Care Information Resource Kits about various types of aged care services and how to access them
  • information about residential aged care facilities
  • information on moving from home to residential aged care
  • support once the move from home into residential care has been made
  • information for staff of residential aged care facilities on cultural, spiritual and language needs of residents from a migrant or refugee background.

Bi-cultural Workers

The Migrant Resource Centre's Community Partners program provides access to our bi-cultural workers who speak Chinese, Vietnamese, Tagalog, Bahasa Malay, Indonesian, French, German, Serbian, Polish, Dutch and Spanish.

Community Support

Our Project Officer can visit communities or groups and give information about:

  • providing the elderly with help to stay in their own home
  • accessing community aged care services in Tasmania
  • accessing residential aged care facilities in Tasmania
  • supporting community members during the transition period between home and aged care facilities
  • involving your community in the lives of members who are living in residential aged care facilities
  • setting up links between your community and appropriate aged care services.

Residential Aged Care Service Providers

For residential aged care service providers who wish to provide a better service for culturally and linguistically diverse communities we can:

  • provide help to identify the cultural, linguistic and spiritual needs of residents
  • provide language and cultural support and assistance - view our downloadable resources
  • assist you to incorporate residents’ language and culture into your programs
  • provide culture-specific cross cultural awareness training
  • facilitate community input into the provision of culturally appropriate care
  • facilitate the creation of networks between your aged care service and communities.

Partners In Culturally Appropriate Care (PICAC)

The Partners in Culturally Appropriate Care program provides free, multicultural concerts, afternoon teas and forums that celebrate culture and bring culturally diverse communities and age care providers together.

Cultural and Language services

The program can help your community or aged care service to meet the cultural and language needs of your members and residents. But it offers much more…………linking community care, Government services and research, the aged care sector and the traditions and culture of both small and large culturally diverse communities in an informal partnership.

Working with aged care providers

PICAC can work with aged care services to:

  • Provide affordable cross cultural awareness training to staff and management
  • Access to resources and information about culturally appropriate care
  • Promote your facility to culturally diverse communities
  • Access staff that understand language and culture
  • Develop and implement a multicultural policy

Working with community organisations

PICAC can assist community organizations by:

  • Providing updated information on aged care
  • Making the Government aware of your needs and concerns
  • Developing a link with appropriate aged care services
  • Developing your communities skills and networks
  • Feedback to aged care facilities the dietary, religious and spiritual preferences of its members
