Aged Care Services
Home and Community Care (HACC)
This program assists people from migrant and refugee (culturally and linguistically diverse) backgrounds. Clients from this program are people who are older or have a disability. Clients also include carers. The program supports people to stay independently in their own homes.
Our Client Workers will assist you with information and access to services that you may need. These may include:
- Housework
- Personal care
- Meals
- Transport
- Social activities
- Medical appointments
- Legal and welfare issues
- Healthy ageing group
- Pontville cemetery visits
Bi-cultural Staff
Our services are supported by bicultural staff who can speak Chinese, Croatian, Vietnamese, Bahasa Malay, Indonesian, French, German, Serbian, Polish, Dutch and Spanish.
Multicultural Day Centres / Seniors Club
Migrant Resource Centre clients are welcome to join our Multicultural Day Centres / Seniors Clubs. Day Centre sessions are held from 10am until 2pm and include activities ranging from entertainment and exercise to interesting information sessions. A hot lunch is included at a reasonable cost and transport can be arranged if needed.
Our current Day Centre sessions are Croatian on Tuesday, German on Wednesday, Chinese on Thursday and Multicultural on Friday
Community Development Officer
Our Community Development Officer will visit communities or groups and:

Provide advice on all aged care services that are available

Listen to your needs and develop an aged care program that is suited to your particular community
Trauma Support
Counselling is available for HACC clients and their families suffering
from torture and war trauma prior to their arrival in Australia.
This Home and Community Care program is funded by Home and Community Care - a joint Commonwealth and State / Territory Program.
Packaged Care Programs
Community Aged Care Packages (CACP)
CACP Packages offer a community based service that aims to help older people to remain living in their own homes by providing flexible services to suit individual needs. Services are provided by a bicultural and bilingual team.
Who is elegible for a CACP Package?
To be eligible, your needs will first be assessed by the Aged Care Assessment Team (ACAT) who will decide whether a care package is suitable for you. The assessment is free. Your doctor, service provider, family member can refer you to ACAT, or you can self-refer.
How much does an CACP Package cost?
Any fees charged for services will form part of the agreement between you and our service. The cost is very reasonable and you will not be denied a service because you can not afford to pay.
A CACP Package can provide services according to what you need and prefer. These may include:
- Case management
- Personal care
- Meal preparation
- Laundry or home help
- Transport - assistance with shopping, visit to doctors or dentists or bill paying
- Social interactions - visit to a relative, friend at home or in hospital, or attend church services or community activities
- Referrals to services you may need
Extended Aged care at Home Dementia (EACHD)
What is the EACHD Package Program?
An EACHD Package is a community based service that aims to help older people to remain living in their own homes by providing flexible services that help to manage the symptoms of dementia. EACHD Packages can also offer approaches and ways to meet the individual needs of clients with dementia when their dementia affects their quality of life.
Who is elegible for an EACHD Package?
To be eligible, your needs will first be assessed by the Aged Care Assessment Team (ACAT) to determine whether an EACHD Package is suitable for you. This assessment is free. The EACHD staff can provide more information to you and any other people that you would like to be involved in your assessment.How much does an EACHD Package cost?
Any fees charged will form part of the agreement between you and our service. If you cannot afford to pay, you will not be denied a service that you need. Our staff can provide you with information about fees.What services can I expect to receive from my EACHD Package?
EACHD Package Services are tailored to suit your individual needs and preferences and may change over time as your needs and preferences change.
Services can include:
- Registered nursing care
- Case management
- Care by an allied health professional such as a physiotherpist, podiatrist or other type of allied care
- Personal care
- Transport to appointments
- Home help
- Meal preparation
- Social interaction
